Patients and their families can be most vulnerable—and least focused—when they receive significant information and instructions from physicians and nurses during a discharge from a hospital stay.
Once they have gotten their family member home, there are many questions that arise because they were unable to focus on the instructions.
Jigsaw provides a solid communication and education opportunity that can include multiple family members as they participate in a live virtual learning program on the disease and treatment required to get their family member healthy again. And, the physician or nurse have a unique opportunity to truly connect with the family, get confirmation of understanding of treatment and answer any questions that arise.
They also have the opportunity to see the patient virtually and assess how they are doing directly after returning home. Jigsaw provides a complete record of education to the family or patient using the built in assessment tools.
Doctors, physician assistants or nurses can review relevant information, instructions and treatment plans using Jigsaw’s video, presentation, image gallery and documents library. With the patient’s permission, they can record the session, so the family can review everything covered during the visit and to mitigate any risk that might arise.
